Use this form to share non-urgent or non-emergency observed student behavior. It is sent to members of IMSA’s Behavioral Intervention Team (BIT Care). This form does not constitute submitting a police report or a general incident report. Bias
Use this form to report a non-urgent incident involving bias. It is sent to the Office of Diversity, Equity and Inclusion.
Academic Misconduct
Use this form to report incidents of academic misconduct. Academic misconduct includes, but is not limited to:
Cheating
Plagiarism
Use of unauthorized sources
Prohibited behaviors noted on a class syllabus
Medical Incident
This form is used to report individuals involved in medical or traumatic situations. It should be used anytime someone seeks or needs medical attention, regardless of transport to the hospital.
