What does/doesn’t my transcript contain?
Your transcript contains your name, social security number (for students graduating/leaving the Academy prior to September, 2005), date of entry, date of exit/graduation, and semester grades for all courses taken at the Academy. It also contains information on mentorship and inquiry projects if applicable.
It does not contain previous school information, standardized test scores, or information about classes taken outside of the Academy.

How can I order a transcript?
You may order a transcript by using the form (PDF, 28k) on our webpage or in writing, with signature release. The release should include:
- Your current name and name under which you were enrolled (if different from current name), address, phone, email & fax. This will be used to contact you should there be a problem in producing the documents you have requested. Email addresses will be used to confirm the mailing of the document;
- Number of transcripts/profiles needed;
- Complete address of where transcripts are to be sent ie: name of person, department, organization, street & building address, city, state, zip;
- Deadline for when transcript needs to be received; and
- Any additional services required (fax, express mail) at additional charges.
How long will it take to process a request?
Unless there is a financial obligation still outstanding at the Academy or some technical glitch, transcript requests are usually processed and mailed the day that the written request is received. Exceptions to this time frame would include heavy volume times such as quarterly report card printings and mailings, mid years (end of January) and end of the year close out processing (end of June).
I’m in a real hurry. Can you take my request over the phone or can I email a request to you?
Federal guidelines require a signature release; therefore, phone and email are not appropriate mediums for transcript requests. You may fax the request as long as it contains a signature release authorizing the transcript request.
How can I reach someone in the Registrar’s/Academic Records Office?

How can I get copies of my standardized test scores (SAT, ACT, AP)?
Test scores are not part of the IMSA transcript. However, if you had scores reported to us we probably have them on file. You may request them using the form (PDF, 28K) from the Registrar or in writing with mailing information, signature release and payment. There is a $5 charge for all test scores.
How can I get copies of my home school academic information?
Contact your home school directly to see what they have available and what their policies are. IMSA no longer reports home school information as part of the IMSA official transcript.
How can I get copies of my immunization records/medical records?
We suggest that you first contact your medical provider/health clinic. If they cannot provide what you need, use the online form or mail/fax the Registrar a request in writing with signature release, all contact and mailing information and payment. Because medical records are not computerized, there is a $25 charge for archival retrieval, copying, and mailing.
What is a “profile”?
A profile is a special folder developed and customized for each individual graduating class. It contains general information about the mission of IMSA, courses, graduation requirements, standardized testing and grade distributions for the graduating class when they were juniors. If in stock, these can be ordered for $3 each. Sorry, we are out of profiles for the Class of 2000.
What does my permanent record contain?
- Final copy of your transcript;
- Attendance records;
- Health records;
- Release of information form for institutional research; and
- Standardized test scores (if reported to us).
How long do you keep my permanent record?
Current Illinois law requires that we keep the documents for sixty (60) years.
Who has access to my permanent record?
Once you leave or graduate, information is only released when you request it in writing. IMSA administrators and contractors also have access for research purposes and for certain legal actions.
